Creating a further holistic and inclusive care planning process with the Helix Centre for the NHS

Project aims:

Inclusively worked with the Helix Centre and the NHS to enable care providers to deliver further holistic care and collaboratively create care plans with both patients and other care providers.

By improving collaboration between care providers, you can deliver a further joined-up health care experience for patients.

 

Approach:

We worked with care providers across Sussex to understand what some of the biggest barriers were currently when trying to deliver holistic care.

We conducted user research with a variety of care providers from care homes, hospices, GP Surgeries, Palliative care teams to third-sector charities and volunteers.

Insights:

There are multiple care plans in multiple places but many care providers cant access them

The user research helped us to understand that currently, many care providers will create a care plan for a patient. For example, a person might have a lung cancer care plan, a mental health care plan and a physio care plan for their injured ankle but they won’t have access to this and all of their care providers wont have access to the other care plans.

This is because it’s often very tricky for care providers to easily share these different care plans due to systems not integrating, data sharing issues and time.

This leads to a lack of holistic care as care providers can’t easily come together to make holistic plans with and for a person.

Impact and outcomes:

We co-created a prototype of how a holistic care planning process could work

We designed an overall ecosystem and service blueprint for a further holistic and inclusive care planning process. We created this based on insights and feedback from two rounds of research and testing with a wide variety of care providers and patients.

We specifically focused on creating improvements that would enable care providers to collaborate further on care plans together and ways for patients to be at the centre of care planning.

The solution developed with care providers and patients across Sussex will be further iterated and tested and possibly rolled out in the future.
